P0269: Cylinder 3 Contribution/Balance Fault

What does P0269 mean?
P0269 is a performance code, not a wiring code. The engine control module (ECM) reads crankshaft rotational speed through the crankshaft position sensor at high resolution. Each power stroke produces a small, measurable acceleration of the crankshaft, and the size of that acceleration is a direct proxy for how much work the cylinder just did. The ECM compares all cylinders and calculates each one's contribution. When cylinder 3 falls outside the allowed spread, P0269 sets.
Because the measurement is of the outcome rather than of any component, three unrelated failure paths produce the same code. Fuel: a clogged, worn, or slow-opening injector delivers less fuel than commanded. Spark: on a gasoline engine, a worn plug or a coil that is breaking down under load produces incomplete combustion. Compression: worn rings, a burnt or leaking valve, or a head gasket leak means the cylinder cannot convert its combustion into pressure on the piston. The crankshaft sensor cannot tell them apart, so the diagnosis has to.
The order in which you test matters more than which tool you use. Compression testing cylinder 3 is cheap, fast, and definitive, and it prevents the most common expensive mistake on this code — installing a new injector into a cylinder that has a mechanical problem. Plug inspection and a coil swap are similarly cheap and rule out the ignition path in minutes. Only after those two come back clean is injector replacement the sensible next spend.
On diesel engines, the picture simplifies considerably. There is no ignition system, so a contribution fault is fuel or compression, and manufacturers build per-cylinder contribution readouts directly into the diagnostics. Worn diesel injectors go slow before they go dead, which is exactly what a balance fault detects — often well before any misfire code appears.
Common causes
- Restricted, dirty, or internally worn cylinder 3 fuel injector
- Injector opening late or delivering a poor spray pattern under load
- Worn spark plug or a failing ignition coil on cylinder 3 (gasoline engines)
- Low compression on cylinder 3 from worn rings, a burnt valve, or a valvetrain fault
- Head gasket leaking between cylinder 3 and a neighboring cylinder or into coolant
- Vacuum or intake gasket leak affecting only the cylinder 3 runner
- Low fuel pressure or a restricted fuel filter showing up first on the weakest cylinder
- Corroded connector or high-resistance wiring at the cylinder 3 injector
Symptoms

Diagnostic steps
- 1.Read all stored codes first. If P0203, P0267, or P0268 are present, the cylinder 3 injector circuit has an electrical fault — repair that and the balance code will usually go with it.
- 2.Confirm which physical cylinder your engine calls number 3 before testing. On V6 and V8 engines the numbering varies by manufacturer and guessing wastes the whole session.
- 3.Run the scan tool's cylinder balance or injector contribution test and record how far cylinder 3 deviates from the others.
- 4.On a gasoline engine, remove and inspect the cylinder 3 spark plug alongside the rest. Heavy wear, fouling, or oil contamination explains the weak contribution on its own.
- 5.Swap the cylinder 3 ignition coil with a neighboring cylinder, clear the codes, and drive. If the fault follows the coil, the diagnosis is finished.
- 6.Compression- and leak-down-test cylinder 3 before spending money on fuel components. This single test separates a mechanical problem from a fuel problem and is the step most often skipped.
- 7.Check fuel rail pressure under load and inspect the fuel filter for restriction — a marginal supply presents as one weak cylinder.
- 8.Listen to the cylinder 3 injector with a stethoscope or run an injector buzz test to confirm it is opening and closing.
- 9.If fuel supply, spark, and compression all test good, swap the cylinder 3 injector with one from a healthy cylinder and see whether the code follows it.
