What does P0281 mean?

P0281 is a report on an outcome, not on a wire. The engine control module (ECM) watches crankshaft rotational speed at high resolution through the crankshaft position sensor. Each power stroke gives the crankshaft a small, brief acceleration, and the size of that acceleration is a direct measure of how much work that cylinder did. The ECM compares all cylinders against one another, and when cylinder 7 falls outside the allowed spread, P0281 sets.

The most useful thing to understand about a contribution or balance code is how it relates to a misfire code. A misfire monitor is looking for combustion events that essentially did not happen — the crankshaft speed drop is large and unmistakable.
