What does P2195 mean?

The upstream oxygen (O2) sensor on Bank 1 sits in the exhaust ahead of the catalytic converter and tells the engine control module (ECM) whether combustion is running rich or lean. In normal closed-loop operation the sensor voltage swings rapidly between roughly 0.1 volts (lean) and 0.9 volts (rich) several times per second, and the ECM trims fuel to keep the average near the ideal air-fuel ratio.

P2195 is different from a circuit code like P0131. It does not mean the wiring is open or shorted — it means the sensor is electrically alive but its signal is stuck or biased toward the lean side and no longer responds normally when the ECM adds fuel. The ECM commands extra fuel expecting the sensor to swing rich, the sensor stays low, and after this mismatch persists the ECM concludes the signal is "biased/stuck lean" and sets the code.

"Bank 1" is the side of the engine that contains cylinder 1; "Sensor 1" is the upstream sensor. The two real-world possibilities are that the sensor itself has aged and lost its ability to swing rich, or that the exhaust really is lean and the sensor is correctly reporting it. Checking the fuel trims separates the two: if long-term fuel trim is already pushed well positive (the ECM piling in fuel) the mixture is genuinely lean; if trims look normal but the sensor won't switch, the sensor is the suspect.

  2. 2.Watch Bank 1 Sensor 1 voltage live on a scan tool. A healthy sensor switches quickly between about 0.1 and 0.9 volts; with P2195 the signal sits low and flat.
  3. 3.Read short- and long-term fuel trims. Strongly positive trims mean the engine is genuinely lean; near-zero trims with a non-switching sensor point at the sensor itself.
  4. 4.Inspect for exhaust leaks ahead of the sensor — cracked manifolds, leaking gaskets, and loose flanges all admit air that reads lean.
  5. 5.Do a smoke test for intake/vacuum leaks and check fuel pressure against spec.
