What does P2198 mean?

This code is the Bank 2 counterpart of P2196. The upstream oxygen (O2) sensor on Bank 2 reports the air-fuel ratio to the engine control module (ECM) from its position ahead of the catalytic converter. In closed loop it should swing quickly between roughly 0.1 volts (lean) and 0.9 volts (rich) as the ECM trims fuel.

P2198 is a rationality fault, not a wiring fault. The circuit is intact, but the Bank 2 upstream sensor signal is stuck or biased toward rich and stops responding when the ECM leans the mixture out. When the voltage stays high despite the ECM commanding less fuel, the ECM flags the signal as "biased/stuck rich" and sets the code.

"Bank 2" is the side of a V-configured engine that does not contain cylinder 1; inline engines have only Bank 1, so P2198 does not apply to them. "Sensor 1" is the upstream sensor. Either the sensor is contaminated or worn and biased high, or Bank 2 really is running rich and the sensor is telling the truth. Because the fault is limited to one bank, a bank-specific cause is likely — a leaking injector on that head, or contamination of just that sensor. Comparing Bank 1 and Bank 2 fuel trims shows whether the rich condition is real and one-sided: strongly negative long-term trim on Bank 2 means the ECM is pulling fuel to compensate for a genuinely rich bank.
